- Valid Japanese passport for all persons concerned AND proof of legal status in the U.S.
◦ Green card, visa, etc.
◦ U.S. Passport or Birth Certificate for dual citizens born in the U.S.
◦ Dual citizens born outside of the U.S. are required to bring their birth certificate known as the Consular Report of Birth Abroad. - An original Family Registry (Koseki Tohon or Koseki Shohon) issued within 6 months of the application date
- If you wish to add information on a foreign citizen included in your Family Registry, we will also need to verify the spelling of their name (Passport, Birth Certificate, etc.)

Additional Information
- The applicant must appear in person to apply. Processing time is 5 business days.
- You may designate a family member to pick up your certificate. They will need your Letter of Authorization
, your passport (copy acceptable), their ID, your pick-up slip and fee in cash.
- If your Family Registry has your name and citizenship before you acquired Japanese citizenship, or your parent-child relationship, you may include it in this certificate.
