Required Documents

  1. Application formOpen a new window
  2. Valid Japanese passport for all persons concerned AND proof of legal status in the U.S.
         ◦ Green card, visa, etc.
         ◦ U.S. Passport or Birth Certificate for dual citizens born in the U.S.
         ◦ Dual citizens born outside of the U.S. are required to bring their birth certificate known as the Consular Report of Birth Abroad.
  3. An original Family Registry (Koseki Tohon or Koseki Shohon) issued within 6 months of the application date
  4. If you wish to add information on a foreign citizen included in your Family Registry, we will also need to verify the spelling of their name (Passport, Birth Certificate, etc.)

Additional Information

  1. The applicant must appear in person to apply. Processing time is 5 business days.
  2. You may designate a family member to pick up your certificate. They will need your Letter of AuthorizationOpen a new window, your passport (copy acceptable), their ID, your pick-up slip and fee in cash.
  3. If your Family Registry has your name and citizenship before you acquired Japanese citizenship, or your parent-child relationship, you may include it in this certificate.
