Birth Certificate (Shussei Shomei)

This English certificate includes your date of birth, place of birth, and the name of your parents based on your Family Registry.

The Family Registry (Koseki Tohon or Koseki Shohon) can only be obtained in Japan. Please contact the municipal office of your Registered Domicile (Honseki-chi) on how to get your Family Registry.

Required Documents

  1. Application FormOpen a new window 
  2. Applicant’s valid Japanese Passport AND proof of legal status in the U.S. 
    • Green card, visa, etc. 
    • U.S. Passport for dual citizens
  3. An original Family Registry (Koseki Tohon or Koseki Shohon)
    • No period of validity required but must have all current life events stated. For example, if you were married in the U.S., your Marriage Registration along with your current name and Registered Domicile must be stated on the Family Registry.

Additional Information

  1. The applicant must appear in person to apply. Processing time is 5 business days.
  2. You may designate a family member to pick up your certificate. They will need your Letter of Authorization,Open a new windowyour passport (copy acceptable), their ID, your pick-up slip and fee in cash.
  3. Japanese citizens, former Japanese citizens, and foreign citizens born in Japan may apply for this Birth Certificate. Instead of a Koseki Tohon or Koseki Shohon, former Japanese citizens will need their Joseki Tohon. Foreign citizens born in Japan will need their “Certificate of Acceptance of Birth Report” (Shussei Todoke Juri Shomeisho).
  4. There are no expiration dates for our certificate. However, the organization you are submitting to may have limitations on validity, so please check with the organization concerned. (E.g., Must be issued within X months.)
  5. If you need information on the Birth Certificate that is not on your Family Registry, please consult our office beforehand.
