Lost or Stolen Passport

If your Japanese passport is lost or stolen, submit the report form and apply for a new passport or an emergency travel document (Tokosho)

Report the Loss or Theft

1. Required Documents
1. One Report of Loss or Theft Form
・ Can be filled out at our office or available to download hereOpen a new window (must type in Japanese and print out the form.) 
・ Use BLACK INK ONLY. Do NOT use an erasable pen.

・ Minors (under age 18) will need a parent or legal guardian signature.
     ◦ If the parent or guardian does not live with the minor, the minor will need the Letter of ConsentOpen a new window from them in order to apply. The applicant must submit the original Letter of Consent along with the envelope it was mailed in.
All applicants (including minors) must appear in person to report the loss or theft.
2. Police report (for theft)
3. Two photos, taken within 6 months
・ To apply for a new passport or travel document (Tokosho) with your report, you will need at least 2 photos.
4. If you have a copy of your lost or stolen passport, please submit it as well.

 

Apply for a New Passport or Travel Document (Tokosho)

・ Processing time is 5 business days.
・ If you have an emergency and need to return to Japan immediately, apply for the travel document (Tokosho).
2. Apply for a travel document (Tokosho). Click hereOpen a new window for required documents.)
・ If you have an emergency and need to return to Japan immediately, this travel document will replace your passport and will be voided once you enter Japan. You cannot enter other countries with this travel document.
・ If you do not have an emergency, please apply for a new passportOpen a new window.
