To Attend Deposition Taking by U.S. Consul |

 DOCUMENTS TO BE SUBMITTED TO THIS OFFICE ARE AS FOLLOWS:
 |
Valid passport (must have a complete blank visa page). |
 |
Two passport-type photos (2” x 2”, taken within six months). |
 |
Two official visa application forms (Original, No copy or fax paper accepted). |
 |
A sealed copy of the COMMISSION (or a sealed copy of the COURT ORDER) for a U.S. Consular Officer to take deposition in Japan. |
 |
The letter requesting a visa, which should be written on letterhead stationary and include the following information: |

The name and location of the Court. |
| |
 |
The name and occupation of each applicant. |
| |
 |
The name of plaintiff and defendant. |
| |
 |
A summary of the case. |
 |
The flight and hotel information. |
As the application requires referral to the Ministry of Foreign Affairs, all necessary documents should be submitted to this office at least two weeks in advance of the intended departure date.
