|
外国の国籍を有する日本国民は、法務大臣に届け出ることにより日本国籍を離脱することができます。届出人ご本人（１５歳未満の場合はその法定代理人）が以下の書類を総領事館に提出してください。

届書はすべて日本語で記入してください。 |

届書はすべて戸籍に記載されている氏名で記入してください。 |
| (3) |
届出は日本国籍を離脱しようとする方の住所地を管轄する総領事館に、必ず届出人本人（または１５歳未満の場合には、法定代理人の資格を証する公的資料を持参のうえ、法定代理人）が提出してください。 |
| (4) |
戸籍謄本には出生時から重国籍の記載のあるものが必要です。もし、その記載がない場合は別途補足資料が必要な場合がありますので、あらかじめご了承ください。 |
| (5) |
法務省において国籍離脱の手続きが完了すると（手続きには２、３カ月かかります）、国籍離脱通知書が交付されます。 |

PROCEDURE IN
RENUNCIATION OF JAPANESE NATIONALITY
BY DUAL NATIONALS
|
 |
Notification
form to be written in Japanese except where English writing
is specifically required. |
 |
Notarized
affidavit signed by the renunciant certifying to the fact
of American citizenship. |
 |
The
Koseki Tohon (or Shohon) issued by the municipal office of
the renunciant’s permanent domicile in Japan dated within
six months of the date of issuance. |
 |
A copy
of passport or certified copy of birth certificate. |
 |
Proof
of address (Driver’s License or mail showing your name and
address).

 |
All
English documents should be translated into Japanese. |
| (2) |
If
a renunciant is 15 years of age or over, he/she will sign
the Renunciation Notification form in the Japanese writing
under the item designated “todokeide nin”, placing right thumb
imprint after the signature. |
| (3) |
If
a renunciant is a minor, under 15 years of age, both parents
or legal guardian should prepare the affidavit of citizenship
for the child and sign the notification papers. |
| (4) |
One
copy of the Koseki Tohon will suffice if there are more than
one member of the family renouncing Japanese nationality at
the same time. |
| (5) |
If
a woman is married and such marriage is recorded in her husband’s
Koseki Tohon but no mention therein of her birth in the United
States, it will be necessary for her to attach an additional
Koseki Tohon of her original family. |
| (6) |
If
the birth dates or the names recorded in the Koseki Tohon
and in the birth certificate differ in any way, additional
notarized affidavit must be attached declaring that the different
dates or names refer to the same person. Such affidavit must
be prepared by either the parents or elder close relatives
who are well acquainted with the renunciant. |
| (7) |
If
the Renunciation Notification is accepted and final action
is taken by the Japanese Ministry of Justice, a procedure
which takes approximately two to three months, the renunciation
will be so notified. |
| (8) |
The
renunciatnt must appear in person to submit the forms. |
