How to Apply for a Visa General Information U.S. Citizen Short Term Stay

DOCUMENTS TO BE SUBMITTED TO THIS OFFICE ARE AS FOLLOWS:

Valid passport (must have a complete blank visa page).
Two passport-type photos (2” x 2”, taken within six months).
Two official visa application forms (Original, No copy or fax paper accepted).
A sealed copy of the COMMISSION (or a sealed copy of the COURT ORDER) for a U.S. Consular Officer to take deposition in Japan.
The letter requesting a visa, which should be written on letterhead stationary and include the following information:
  The name and location of the Court.
  The name and occupation of each applicant.
  The name of plaintiff and defendant.
  A summary of the case.
The flight and hotel information.

As the application requires referral to the Ministry of Foreign Affairs, all necessary documents should be submitted to this office at least two weeks in advance of the intended departure date.
